Fees

My fee is $100 for a one hour individual session and $125 for family or couples counseling. I am an in-network provider for Aetna, Ceridian, Cigna, Humana (through LifeSynch), Guardian, Trustmark, and Private Health Care Systems (PHCS/MultiPlan) health plans. I am an out-of-network provider with Blue Cross/Blue Shield, which means my services are covered after your annual out-of-network deductible (usually $500) is met. I will provide a receipt for services rendered that you can submit to other insurance carriers for reimbursement.

Getting Started

If we decide to work together you may wish to read, download, and sign the Client Information Sheet that follows and complete the intake forms. We can discuss these as well as the release of information in our first session and I will answer any of your questions .

The Consent to Treatment forms briefly explain the nature of therapy, limits to confidentiality, and my policies and procedures. Your signing it indicates your consent to treatment.

The intake forms provide me with basic background information so we can spend the session talking about what's bothering you rather than you answering a lot of questions. If possible, please download, complete, and bring to your first session.
